Follow the below steps to request a signature on a form:

Click on "Request Form Signature" under the "Forms and Documents" section of the left hand navigation.

Select the form you would like to submit for signing from the dropdown (A), enter in the name of the employee you want to send to (B), and click "Show Form" (C).

Confirm that it is the correct form, and then click "Populated Form".

Any employee information that is already in the system will then be pre-populated into that form.  Any additional information that is required on the form will be filled in by the employee when they review and sign. 

Once the form has been populated, click "Submit for Signing"

That employees enrollment status will then be "Signature", meaning pending signature, and the employee will be sent an email from the Maxwell portal, notifying them that there is a form that needs their signature.
